Group (Society) Officer Report Forms - For Group (Society) Presidents/Secretaries - Complete the following form by January 15 to report your group officer listings to the Zone secretary* annually.

Group (Society) Officer Compilation Report Form - For Zone Secretaries - Complete the following form by February 1 to report the group officers in your zone to the LWML SD District secretary annually.
